It is perhaps most useful to approach the Doctor-Patient relationship (DPR) by admitting that it’s complicated. We review some of the strategies that have been employed to mitigate this complexity, zeroing in on one that promises to capture the main features of the DPR without eliminating some of its more important, existential components; pieces of the puzzle that must be retained if we are to avoid oversimplification and the errors that can arise by ignoring important foundational properties. We believe that (...) a useful way to look at the DPR and to capture essential features that must be balanced in the process is provided by Partnership Theory and its definition in terms of the so-called domination and partnership systems. We apply this theory to the DPR and investigate the implications of this application to health care. We see that in the absence of mitigating circumstances, adoption of the patient-as-partner model serves healthcare well and is flexible enough to accommodate circumstances that dictate modifications. This book presents an alternative to conventional ideas about the evolution of the human intellect.

"Intelligence" has long been considered to be a feature unique to human beings, giving us the capacity to imagine, to think, to deceive, to make complex connections between cause and effect, to devise elaborate stategies for solving problems. However, like all our other features, intelligence is a product of evolutionary change. Until recently, it was difficult to obtain evidence of this process from the frail testimony of a few bones and stone tools. It has become clear in the last 15 (...) years that the origins of human intelligence can be investigated by the comparative study of primates, our closest non-human relatives, giving strong impetus to the case for an "evolutionary psychology", the scientific study of the mind. To explain social learning without invoking the cognitively complex concept of imitation, many learning mechanisms have been proposed. Borrowing an idea used routinely in cognitive psychology, we argue that most of these alternatives can be subsumed under a single process, priming, in which input increases the activation of stored internal representations. Imitation itself has generally been seen as a This has diverted much research towards the all-or-none question of whether an animal can imitate, with disappointingly inconclusive results. Representing and manipulating relations between objects seems to be one basic building block in their hierarchical programs. There is evidence that great apes suffer from a stricter capacity limit than humans in the hierarchical depth of planning. We re-interpret some chimpanzee behaviour previously described as and suggest that all great apes may be able to imitate at the program level. Investigators of animal behavior since the eighteenth century have sought to make their work integral to the enterprises of natural history and/or the life sciences. In their efforts to do so, they have frequently based their claims of authority on the advantages offered by the special places where they have conducted their research. The zoo, the laboratory, and the field have been major settings for animal behavior studies. Just as biologists have their favored places for doing research, so do historians. As someone who likes working in archives, the most surprising thing the present author ever found was a particular letter that had been written to him by the ethologist Niko Tinbergen—but that Tinbergen had never sent. The letter included a detailed critique of the intellectual style and conceptual shortcomings of Tinbergen’s career-long friend and colleague Konrad Lorenz. The present author first saw the letter 3 years after Tinbergen’s (...) death and 10 years after the letter was composed. Here we discuss the contents and historical context of that letter. (shrink)

Comparative analysis of the behavior of modern primates, in conjunction with an accurate phylogenetic tree of relatedness, has the power to chart the early history of human cognitive evolution. Adaptive cognitive changes along this path occurred, it is believed, in response to various forms of complexity; to some extent, theories that relate particular challenges to cognitive adaptations can also be tested against comparative data on primate ecology and behavior. This paper explains the procedures by which data are employed, and uses (...) the best currently available evidence to derive a proposal for some of the stages through which human cognition evolved, before the last common ancestor with a nonhuman, and the reasons that cognitive adaptations were favored in primate evolution. This dissertation interprets the lack of uniqueness in probability representations of agents' degrees of belief in the decision theory of Richard Jeffrey as a formal statement of an important epistemological problem: the underdetermination of our attributions of belief and desire to agents by the evidence of their observed behaviour. A solution is pursued through investigation of agents' attitudes to information of a conditional nature. ;As a first step, Jeffrey's theory is extended to agents' conditional attitudes of belief and desire (...) by introduction of a definition of conditional desirability. The second step is an analysis of agents' attitudes to conditionals. Following Adams it is argued that the probability of a conditional is the conditional probability of its consequent given the antecedent. Since numerous 'trivialization' results show that no proposition could be such that this were the case, it is proposed that conditionals express a variable proposition as a function of the context in which they are asserted. The consequent technical treatment of conditionals as proposition-valued functions of sets of possible worlds delivers Adams' thesis as a natural consequence without fear of trivialization. ;Conditionals are the underlying objects of the decision theory that is presented in the final chapter. A representation theorem is proven for preferences defined over sets of such conditionals satisfying certain closure conditions, where an agent is understood to prefer one conditional over another just in case she prefers that the former by true rather than the latter. The theorem establishes that a rational preference ranking of conditionals implies the existence of a unique probability function and a desirability function unique up to choice of scale, that agree with the preference ranking, and that these functions satisfy both the axioms of Jeffrey's decision calculus and Adams' thesis. The theorem is interpreted as saying that elicitation of an agent's choices amongst conditional options suffices to determine our attributions of belief and desire to them. (shrink)

Imitation research has been hindered by (1) overly molecular analyses of behaviour that ignore hierarchical structure, and (2) attempts to disqualify observational evidence. Program-level imitation is one of a range of cognitive skills for scheduling efficient novel behaviour, in particular, enabling an individual to purloin the organization of another's behaviour for its own. To do so, the individual must perceive the underlying hierarchical schedule of the fluid action it observes and must understand the local functions of subroutines within the overall (...) goal-directed process. Action-level imitation, copying strings of actions linearly without any such understanding, is less valuable for acquiring complex behaviour and may often have other, social functions. At present, we lack a mechanistic understanding of the abilities underlying program-level imitation that make it possible for the underlying structure of complex actions to be dissected visually and recreated in behaviour. In a wide-ranging inquiry Richard W. Miller provides new resources for coping with the most troubling types of moral conflict: disagreements in moral conviction, conflicting interests, and the tension between conscience and desires. Drawing on most fields in philosophy and the social sciences, including his previous work in the philosophy of science, he presents an account of our access to moral truth, and, within this framework, develops a theory of justice and an assessment of the role of morality in (...) rational choice. In Miller's view, we are often in a position to claim that our moral judgments are true descriptions of moral facts. But others, relying on contrary ways of moral learning, would reject truths that we are in a position to assert, in dissent that does not depend on irrationality or ignorance of relevant evidence or arguments. With this mixed verdict on "moral realism," Miller challenges many received views of rationality, scientific method, and the relation between moral belief and moral choice. In his discussion of justice, Miller defends the adequacy, for modern political choices, of a widely shared demand that institutions be freely and rationally acceptable to all. Drawing on social research and economic theories, he argues that this demand has dramatically egalitarian consequences, even though it is a premise of liberals and conservatives alike. The powerful combination of genetic, developmental and molecular approaches possible with the fruit fly, Drosophila melanogaster, has led to a profound understanding of the genetic control of early developmental events. However, Drosophila is a highly specialized long germ insect, and the mechanisms controlling its early development may not be typical of insects or Arthropods in general. The beetle, Tribolium castaneum, offers a similar opportunity to integrate high resolution genetic analysis with the developmental/molecular approaches currently used in other organisms.
